Abstract

We present a novel formulation of Sildenafil (Viagra) for oral administration in a single dose (Sildenafil in doses ranging from 25 to 100 mg/100 g) in male subjects with an age range of 24 to 39 years. The Sildenafil formulation is a potent inhibitor of cyclooxygenase-2 (COX-2) which plays a major role in the production of prostaglandins and prostaglandin D2 (PGD2), two mediators of inflammation and pain. Our recent report indicates that Sildenafil is an inhibitor of cyclooxygenase-2 (COX-2) in male subjects, suggesting that Sildenafil may be a potential therapeutic agent for treatment of pain disorders.

Introduction

The pathogenesis of pain is a complex process involving the synthesis and release of chemicals in the brain and spinal cord. The primary function of the brain is to regulate the activity of many neurons and to control various physiological processes, such as pain, inflammation and fever. Pain is a complex and multifactorial condition, and it can vary widely from one individual to another. The pain response to injury, disease and injury severity is dependent on various factors including the underlying pathology, genetic susceptibility and underlying genetic background.

In the early stages of pain, the brain receives multiple signals from various sources, including the sensory nerves, musculature, and visceral nerves. These signals are integrated and sent from the sensory system to the central nervous system, where they are then activated by various neurotransmitters such as acetylcholine, dopamine, noradrenaline and norepinephrine, and finally activated by histamine, serotonin and serotonin metabolites. Phosphodiesterase type 5 (PDE-5) is an enzyme responsible for the breakdown of cyclic guanosine monophosphate (cGMP), which is the main physiological molecule involved in pain. Phosphodiesterase type 5 (PDE-5) activity is significantly increased in the spinal cord in many cases and can be inhibited by the phosphodiesterase type 5 inhibitor (PDE-5i) such as sildenafil (Viagra).

Sildenafil, the active ingredient in Sildenafil (Viagra), has been shown to potentiate the pain-relieving effects of various analgesic drugs. The mechanism of action of Sildenafil is attributed to its ability to inhibit cyclooxygenase-2 (COX-2) in the spinal cord and the inhibition of the enzyme-specific enzyme thromboxane-specific phosphodiesterase type 5 (TPE-5) in the lung. The mechanism of action of Sildenafil is attributed to its ability to block PDE-5 at the site of inflammation. The inhibition of COX-2 is known to play a major role in the activation of PDE-5 in the CNS, and it has been shown to be a significant factor for the production of prostaglandins and PGE2 in the CNS. In this study, we demonstrate that the Sildenafil formulation, as an oral drug, can be used as an effective and well-tolerated option in patients with mild-to-moderate pain, where Sildenafil is an inhibitor of COX-2 and is also associated with an increase in the prostaglandin and PGE2 levels in the serum and brain. The Sildenafil formulation has a high penetration rate of the drug in the oral area (60-fold), a relatively long half-life (12 hours), and a high bioavailability, which enables it to be taken orally as a single dose. In addition, the drug is well tolerated by the patients. This drug is also well tolerated by the patients and has a good safety profile.
